Powers of Attorney for Health Care

The purpose of the Power of Attorney for Health Care is to provide the person you designate (your “agent”) broad powers to make your health care decisions. The Power of Attorney form does not impose a duty on your agent to exercise granted powers, but when the powers are exercised your agent will have to use due care to act for your benefit. The Illinois Power of Attorney for Health Care Act requires that the agent keeps a record of all receipts, disbursements and significant actions taken as an agent.
